Forward Wellness Cancellation Policy

To ensure the efficient management of our schedule and to respect the time of our practitioners, we have implemented the following cancellation policy:

Card On File: We require all patients to provide a credit card when scheduling an appointment. In the event of a late cancellation or no-show, the card on file will be charged the applicable fee.

Cancellation Notice: We require a minimum of 24 hours' notice for appointment cancellations or rescheduling. This allows us the opportunity to offer the appointment slot to another client who may be waiting.

Less than 24 hours' notice: In the event of a cancellation without 24 hours' notice, a fee of $50 will be charged to the credit card on file.

No Show: A 'no-show' appointment refers to when a patient fails to arrive for a scheduled appointment without prior notice. In the event of a no-show, the patient's credit card on file will be charged a fee of $100.

Exceptions: We understand that emergencies and unforeseen circumstances can occur. Exceptions may be made on a case-by-case basis at the discretion of management.
